Pantoprazole is a substituted benzimidazole that inhibits the H+/K+ ATPase enzyme in gastric parietal cells, blocking the final step of acid secretion. It suppresses both basal and stimulated gastric acid production.
Controlled-release: Take on an empty stomach, 1 hour before meals. Swallow whole; do not chew or crush.
Standard-release: Can be taken with or without food.
Erosive esophagitis: 40 mg once daily for 8–16 weeks; maintenance 40 mg once daily.
GERD (short-term IV): 40 mg IV once daily for 7–10 days, switch to oral when possible.
Zollinger-Ellison syndrome: 40 mg once daily PO; up to 240 mg/day in some cases. IV: 80 mg every 8–12 hours, up to 7 days.
Duodenal ulcer: 40 mg once daily for 2–4 weeks.
Gastric ulcer: 40 mg once daily for 4–8 weeks.
Pediatric Dose:
<5 years: Safety not established.
5 years (15–40 kg): 20 mg once daily for up to 8 weeks.
≥40 kg: 40 mg once daily for up to 8 weeks.
Elderly & Renal Impairment: No dosage adjustment usually needed.
Hepatic Impairment: Max 20 mg/day or 40 mg on alternate days.
Headache (>4%), abdominal pain (4%), chest pain (4%), diarrhea (4%), rash/pruritus (4%), vomiting (>4%), constipation (<4%), facial/generalized edema (<4%)
Other possible: Angioedema, atrophic gastritis, optic neuropathy, hepatocellular damage, interstitial nephritis, pancreatitis, pancytopenia, rhabdomyolysis, anaphylaxis, Stevens-Johnson syndrome, toxic epidermal necrolysis, erythema multiforme
